Current pricing on GV units are $2600 to $2800 depending on the model / application. I picked the unit I used up a few years ago when they were about 300 less.http://www.gearvendors.com/prices.html

Custom drive shaft fabrication work will run around $150 per drive shaft.

Labor on this was mine, along with fabrication of the mounting plates and custom 1350 yokes to interface with the stock transmission and differential flanges.

If I had to have all this work done by someone else it would have been around $5000 - $6000.
