The chrome cap is most likely a lock disguised as hub cap. These were available for various makes of cars with spares that were carried outside such as sidemounts. I have one a little smaller than that one.

The reinforced cowl as HH56 indicates was most likely due to the fact that FDR had Poliomylitis and could not stand nor walk on his own.
