What happened to me:

The slot stripped out and I couldn't get it out. I cleaned out the threads in the carb body after I got the jet pulled out.

What the picture doesn't show is where the needle portion of the jet became detached from the piece with the external threads. I believe this happened when I removed the jet- I had to drill it out with a 3/32 drill bit and use a reverse threaded screw extractor. I don't think that I could reuse this jet because it's now in two pieces.
