I just realized I had a scan of another card on file. Here is a reproduced image. It is as exact as one could possibly get. I made sure each letter was spaced exactly as the original.
If anyone would like to have a hi-res pdf so that they can print it out onto card stock, just send me an e-mail.
The black border around the edge is just for size and to show the rounded corners of the original. Same for the round circles at the top, depicting where the string hole would be.
I'm told that the instructions are the same whether for R-9 or R-11.
