When I have seen this is when the crankshaft comes back with the mains at 0.002 or so under the correct size, due to polishing or clean up. The same with the Camshaft. Often when a crank or cam comes back from a trusted supplier, the rebuilder does not re-measure it, but trusts that it is correct. When this is discovered, bad words have been known to be uttered!
