For #3, look a few inches in from the end of the tailpipe right at the corner above the bumper brackets where the rear of the frame and rear cross brace join next to the gas tank. Maybe it is just the angle of the photo but your tail pipe looks a bit more to center of the car than mine is. The little arm offsets the hanger a few inches from the holes in the corner of the frame so the hanger lines up with the tailpipe.

For number 4 which goes next to the silencer it appears to be supported in the middle of a piece of the rubber insulating strap material which spans a distance over the tailpipe. While I can't see much without raising the car, I believe one end or maybe both ends fasten to the body. There looks to be some kind of reinforcing plate holding the insulator strap to the body but possibly only one end fastens to the body and the plate supports the other end of the insulating strap. I couldn't see or get my hand in to feel if there is a second bolt.
