Swop it to a different wheel with a tight cap and see if the same looseness goes with it or it becomes tight and the swopped cap rotates on that same wheel.
That'll eliminate the wheel or the cap as the cause of your troubles. Then you can proceed to a solution.

If it has "taken an unauthorized ride" at some time, as alluded to, it is probably out of round, bent, and/or rim damaged. But can't tell from here. My eyes aren't that good. There are people that repair hubcaps and wheels, but not up on that at moment.
