Heli-Coil Usage

Hello all, I'm looking for advice on using a heli-coil insert on my 327 straight eight for a head stud. While torqueing the head down one of the studs pulled straight up out of the block. Upon further inspection I noticed that the stud was different than the rest, leading me to believe that it has been changed before. Thanks in advance for any info. Ramon
