You would use the items listed for the 5588 / 5688. G440710 screw #14 x 5/8 tapping crosses to the one shown. Straight slot probably as that was common elsewhere but mine is philips so may have been changed. What I cannot help on is the insulating washer. Mine only has screws & regular washers so maybe O_D or Randy know what where or which side that goes.
