I've got a scan of just one page (sidemount mirrors) and I'll add it here when I get back to my PC.
PS - the mirror for Packards with metal-cover sidemount covers as pictured at the lower left of this page is now in reproduction at American Arrow based on an original which I have loaned them for this project.
