1951 Packard Owner's Service Policy

I came across this today which is the Packard Owner's Service Policy from one of my Henney-Packards. In this case, it was a 1951 combination. Does anyone know which years used this same policy design and text? I want to make a few for myself and if someone can supply a scan of a better copy, I would appreciate it.
