Have you contacted the authors about availability? That would be the first step if you want your own copy.
I have a copy of the publication and Don Reedy graciously gave me permission to scan and post from it so I should be able to do that for your car tomorrow. EDITED to add scans. Attach file:  (91.62 KB) (59.63 KB)
