Is this a common occurrence?

All towards the rear of the car, FWIW....

About as common as sloppy mechanics are common.

Probably someone removed them because they were clogged, couldn't clean them and didn't have any new ones, so left it as it was.

Can't speak for common but a couple of mine pulled off when I greased a while back. Some locations apparently are just press fit and not screwed in so they probably could fall out once they were loosened by whatever. In my case, the grease gun didn't want to let go of them so pulling the grease gun off pulled the fitting out. I put mine back in but no guarantee they will stay now.
