It may be that the lock nut is not tight enough against the backing plate. The nut could be bottoming out before gets tightened against the plate. Is there room for a flat washer? If so, this may take up space enough for the nut to tighten.
Is the other side doing the same thing? If not, look at it and see if there are any differences. I don't know, just a guess.

There is a thick lock washer there now. I'll look today to see if there is room for a flat or if the nut could bottom out. I am going to replace the existing lock washer with a fresh one in case it is "tired".
