Brake ecentrics

Overhauled my 39 Six, engine runs great, brakes, not so great. Rebuilt backing plates, new shoes, springs, wheel cylinders, bearings seals and shoe clips. Brakes were pretty good for almost 1,000 miles but recently, brakes "howl" / squeak. Pedal seemed to get softer as well. Bled brakes again, no air.I took off the left front and found eccentric had backed away. (not the star wheel adjustment at bottom, the eccentric adjuster at about the 10:00 or 2:00 position on the backing plate). Even though it has a locking nut, can't seem to get it tight enough as after I adjust it, just a few miles later, it has backed off.
SO, looking for suggestions and, a source for one these eccentrics as I broke the the small square flange for a wrench off. I did drill the core, tapped it and put a hex bolt in, kinda works, but know I really need to cinch these down tighter

It may be that the lock nut is not tight enough against the backing plate. The nut could be bottoming out before gets tightened against the plate. Is there room for a flat washer? If so, this may take up space enough for the nut to tighten.

Is the other side doing the same thing? If not, look at it and see if there are any differences.

I don't know, just a guess.

There is a thick lock washer there now. I'll look today to see if there is room for a flat or if the nut could bottom out. I am going to replace the existing lock washer with a fresh one in case it is "tired".

Thanks for the thought
