It would appear that mount for the driver's side shackle is a bit crooked. Nothing loose, and the frame appears straight. Has anyone run into this sort of thing before? Driver's side is nice and straight.

Perhaps the shackle bushing on one side is toast?

When I first noticed it, a bad shackle is what I thought too, but here's another picture without the filler tube in the way. (My square is on the side of the frame.) The spring is twisted from the rear axle to the rear shackle. The twist has messed up the bushings.

I'm going to have a closer look at the bracket next time it's up on the hoist, maybe replacing the bracket would correct this?

The passenger's side is pretty straight.

Did you check to see that the side and bottom of the frame are square where the bracket is attached? (It looks like the square you are using would be too big.)
How are the brackets attached?
