It is self-published so the easiest way is just to call Reedy or Shaub and order it, they are both PAC members and listed in their Directory. It's more of a looseleaf binder than a book, I bought mine a couple of years ago and I believe I have gotten 3 sets of undated pages - makes it more like a subscription. If you're unable to find a phone # for either, send me a PM.
