There are installation instructions in the accessories section of 51-4 service manual which may be of some help. Hard to see but the holes in your molding don't look quite like the instructions say to do.

Believe the instructions are saying to make a hole thru molding and door panel and another hole thru molding only. That second hole is used as a locator or anti-twist. The next step drilled a smaller hole abutting the first or in effect making a keyhole slot. The missing widget looks to be a setscrew type arrangement with a moveable washer permanently fixed on one end. That item screwed into the mirror. It's allen head was visible and accessible from the outside thru the threaded hole in mirror base.

The mirror was set in place and manipulated until the washer went into the hole thru doorpanel and then pulled toward the narrow part of slot. Once there, the mirror was aligned and held while the setscrew was turned backward thru the hole in mirror base. That would be unscrewing in the conventional sense but would actually be pulling the washer in against the door panel and in turn pulling the mirror in toward the molding and fixing it in place.

The screw/washer arrangement has been made by some by getting a proper size and length ordinary setscrew and welding a washer on it's end. Downside is that method doesn't let the washer or setscrew move independently so it's a little harder to maneuver into the hole and tighten the setscrew.

What the heck is that hole on the front edge of the mirror stem?

My original mirrors do not have that hole.
