This is a pretty awful photo, but the swivel plate goes on top of the chassis plate. The bolt that holds the swivel plate to the chassis goes in the indentation. I'll try to take a better shot if this isn't of any help.

These were all taken from the top down, passenger side. Which would indicate to me that you've currently got your swivel bracket upside down.
