Not 100% Guscha. But if you look at the "W" in COMMONWEALTH You can see the same slant on the sides of the W.

Why the 2 street address?
Most likely reason is that the car was shipped by rail and the showroom on Commonwealth didn't have a rail access. Railroad Auto Service was a company with rail access and equipped to handle offloading of cars, probably for dealers of many brands who shipped cars by rail.

was sale means that once it left the factory that no further changes can be made?? Warranty As Shipped! just guesing Charlie
